Join Licking County Board of Developmental Disabilities as a Transition Youth Service Coordinator and make a meaningful impact in the lives of young people with developmental disabilities. In this role, you will support individuals aged 13–22 in identifying and achieving their personal goals and aspirations. You will collaborate with families, caregivers, educators, and service providers to coordinate person-centered supports and develop comprehensive Individual Service Plans (ISPs) that lead to fulfilling, independent lives.
Key Responsibilities:
- Assess the needs of individuals and identify desired outcomes through person-centered planning
- Develop and implement effective Individual Service Plans (ISPs) for youth aged 13–22
- Coordinate services and refer to appropriate community resources and supports
- Collaborate with families, schools, and other stakeholders to support the individual’s life goals
- Monitor progress and adjust service plans as necessary
- Advocate for the rights and needs of individuals with developmental disabilities
- Maintain accurate and timely documentation and records
Minimum Qualifications:
- Associate’s Degree required; Bachelor’s Degree preferred
- Strong organizational skills and attention to detail
- Ability to manage multiple responsibilities and meet deadlines
- Excellent communication and interpersonal skills
- Passion and enthusiasm for empowering youth with developmental disabilities
- Ability to work independently and as part of a collaborative team
