Position Description

The Adult Team Service Coordinator plays a vital role in supporting adults with developmental disabilities to live fulfilling lives and pursue their goals. This position involves assessing individual needs, coordinating appropriate services, creating and maintaining Individual Service Plans (ISPs), and connecting individuals to community resources.

Key Responsibilities:

  • Assess individual needs and determine desired life outcomes
  • Develop and implement Individual Service Plans (ISPs)
  • Coordinate and refer services from community providers
  • Support adults in achieving independence, inclusion, and personal goals
  • Maintain documentation, meet deadlines, and manage case records effectively

Eligibility Criteria

Minimum Qualifications:

  • Associate’s Degree required
  • Bachelor’s Degree preferred

  • Strong organizational and detail-oriented skills
  • Ability to meet deadlines and manage multiple responsibilities
  • Passion for helping individuals with developmental disabilities
